Patient Forms
Enjoy filling out our short and simple forms during your visit to our office. We have a dedicated kiosk where you can provide us with information via our computer. This information is linked to our system so we can access it speedily and securely.

Insurance Plans
We are happy to help you with your claims.
We accept the majority of insurance plans and are happy to process your claim. Please bring the relevant information with you to your first appointment.
Insurance policies commonly cover only a portion of the full treatment cost. We ask that you pay your portion of the bill at the time of treatment unless arrangements have been made separately. It is your responsibility to pay any balance not paid by your insurance company.

What to Expect from Aesthetica Contemporary Dentistry
As a new patient, you are welcomed as one of the family and treated like our VIP.
You will be warmly greeted and taken to our interactive computer kiosk. Here we will guide you through your digital new patient forms and lead you through to the treatment room. Our team then takes a full mouth series using digital x-rays and intraoral cameras.
While you're in the chair, our dental assistant caters to your needs and takes photos of your entire mouth. These photos are used to assess your health and educate you on the progress you've made.
Dr. Kollia or Dr. Fu will walk you through a new patient exam and discuss the x-rays and images we took earlier. They will explain their findings and make recommendations about the best course of treatment for you.
Finally, we create an electronic treatment plan so you know what our treatment recommendations are, and can set your goals accordingly.
